Advanced Practice Registered Nurses / Physician Assistants
Mott Children's Health Center -a privately funded not-profit organization located in Flint, Michigan- is seeking a full-time Pediatric or Family Nurse Practitioner to join their Child & Adolescent Primary Care Clinic.
MCHC supports the Patient-Centered Medical Home (PCMH) by working with patients, families, providers & staff to promote timely access to needed care; by providing daily continuity of care coordination; and by educating patients, families & caregivers to understand the patient's care plan and self-care management responsibilities.
Essential Job Functions:
Provides primary & preventive health services in collaboration with and under the supervision of a physician.
Performs physical & psychosocial assessments of our child/adolescent patients and counsels patients/families of health & illness.
Electronically documents assessment data concisely, accurately, completely & timely.
Coordinates with community providers/resources to make referrals as appropriate and to follow-up if needed.
Assists in planning & presentation of interdisciplinary team educational conferences.
Assists with development/implementation of Evaluation & Quality Improvement and/or Enhancement Plan for assigned programs.
Provides direct clinical supervision of assigned staff in the clinical setting.
Implements MCHC policies/procedures and assists in the development of new policies/procedures.
Graduation from an accredited School of Nursing and Nurse Practitioner program.
Current Michigan Registered Nurse license and Nurse Practitioner license.
Pediatric Nurse Practitioner or Family Nurse Practitioner certification.
Current CPR certification.
Intermediate computer skill level required in EHR/EMR/EDR.
Basic computer skill level required in Microsoft Outlook/Word/Excel/PowerPoint/Access, Remote Desktop and Adobe Acrobat.
