Connecticut Children's Medical Center is nationally recognized as the only free standing hospital in New England dedicated exclusively to the care of children and adolescents in a patient-and family-centered environment with a focus on research, education and advocacy in Connecticut. In addition to 187 inpatient beds, our services include a state-of-the-art ambulatory surgery center, five specialty care centers and ten practices across Connecticut. Our Level 1 Pediatric Trauma Center is the busiest between Boston and New York. Connecticut Children's has 2,600 employees with a medical staff of more than 1,000 practicing in general pediatrics and more than 30 medical and surgical subspecialties. The Connecticut Children’s St. Mary’s location in Waterbury is a 12-bed unit staffed by our Inpatient Management Team physicians.
This role is within the Perioperative Service Area. We could not do what we do every day without this skilled team and would love for you to be a part of this amazing journey! The team cares for 35-60 children a day. The scope of practice spans from quick ear tube procedures to complex orthopedic and neuro procedures.
The Advanced Practice Registered Nurse Practitioner (APRN)/Care Coordinator is accountable for delivering comprehensive quality patient care, including diagnostic and therapeutic interventions according to standards of practice for children from birth through adolescence. The Pediatric Nurse Practitioner/Care Coordinator functions as a member of the Perioperative Services team.
Essential Duties & Responsibilities:
Treats and evaluates patients with childhood illnesses and promote the health of the child and family.
Performs pediatric histories and physical examinations; interprets diagnostic lab test and x-ray results.
Consults with attending physicians to decide patient referrals and monitor outcomes.
Documents care clearly and accurately within the medical record.
Demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families.
Coordinates patient care for complex, high risk children with the perioperative team, other departments, community agencies and providers as necessary.
Participates in special programs and services as assigned.
Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CCMC STANDARDS.
MSN Required, with minimum of 2 years of pediatric experience required
Bilingual, bicultural in English and Spanish preferred.
Ability to manage complex family issues and provide care coordination between the outpatient arena and Perioperative Services.
Current State of Connecticut Advanced Practice Registered Nurse (APRN) licensure required.
