Advanced Practice Registered Nurses / Physician Assistants
Mott Children’s Health Center (MCHC) in Flint, Michigan is looking for a full-time Pediatric or Family Nurse Practitioner.
General Summary of Job Duties: Provides primary and preventive health care service to children and adolescents. Performs physical and psychosocial assessments of patients. Collaborates with physicians and other professionals in managing care of patients. Supports the patient-centered medical home (PCMH) by working with patients, families, providers, and staff to promote timely access to needed care, providing daily continuity of care coordination, and educating patients, families and caregivers to understand the patient’s care plan and self-care management responsibilities and documents the same in electronic health record. The nurse practitioner works as a member of the care team. There is an ongoing interaction of team members to discuss roles, responsibilities, communication and patient care coordination among team members, working together to provide and enhance the care provided by the practice. Provides direct clinical supervision in the clinical setting and/or of other assigned staff.
Essential Job Functions:
Provides primary health care to children/adolescents in collaboration with and under the supervision of a physician.
Counsels patients and families on the physical and psychosocial aspects of health and illness.
Electronically documents assessment data concisely, accurately, completely and in a timely manner.
Evaluates findings and assists patients/families in developing a health care plan and collaborates with other health care team members as needed.
Identifies community resources for patients/families and makes referrals and provides follow-up as appropriate.
Coordinates with outpatient clinics, subspecialty clinics, home/inpatient services and provides follow-up as needed.
Assists in planning and presentation of interdisciplinary team educational conferences.
Provides direct clinical supervision of assigned staff in the clinical setting.
Assists with developing and implementing an Evaluation and Quality Improvement and/or Enhancement Plan for assigned programs.
Assists in the development of new policies and procedures. Implements existing policies and procedures.
Attends internal and external educational programs and reads appropriate professional journals to maintain and enhance nursing skills and clinical competencies.
Assists the Director, Child & Adolescent Health Services with program/department activities and assignments as needed.
In collaboration with the Director, Child & Adolescent Health Services, participates in clinic staff interviews and provides orientation/training for clinical staff as requested.
Oversees the ordering of medical supplies, medical equipment and medication, if needed.
Graduation from an accredited school of Nursing and Nurse Practitioner program
Licensed in the State of Michigan as a Registered Nurse and a Nurse Practitioner
Pediatric Nurse Practitioner or Family Nurse Practitioner certification and current CPR certification required
Must have a high degree of communication and interpersonal skills to effectively interact with children, adolescents, parents, community agency personnel, administration, associates and visitors
Basic computer skills required in Microsoft Word, Excel, PowerPoint, Outlook, Access, Remote Desktop, Adobe Acrobat
Intermediate computer skills required in EHR/EMR/EDR
