Department/Unit Summary
The Department of Pharmaceutical Services provides comprehensive pharmaceutical care services to Vanderbilt University Hospital (VUH), Monroe Carell Jr. Children's Hospital at Vanderbilt (Children's Hospital), Vanderbilt Psychiatric Hospital (VPH) and The Vanderbilt Medical Group (VMG).
This patient-centered service is provided through the Patient Care Center organization and clinic structure.
The role of the Pharmacy is to promote safe and effective medication therapy for all patients and to provide oversight and leadership of the medication use system.
The Vanderbilt Transplant Center, one of the largest organ transplant programs in the Southeast, currently offers vibrant programs in liver, kidney, pancreas, heart, lung, and bone marrow transplantation. For more than 50 years, beginning with the first transplant in 1962, we have helped restore more than 8,500 people to health. The team performed more than 500 organ transplants during the last fiscal year. Outcomes across our programs meet or exceed national standards. Your Role:
This Clinical Pharmacist will be a member of the Transplant Clinical Pharmacy Team. This team provides care in a variety of areas, including renal/pancreas, liver, lung, heart, and pediatrics. This pharmacist will be one of three Heart/Lung Transplant Clinical Pharmacists. Pharmacists are an integral aspect of patient care at Vanderbilt, with a strong presence and excellent relationships with physicians and nursing partners across all specialties. Collaboration and integration with pharmacy, nursing and medical staff, and excellent communication skills are essential. This pharmacist will have the following clinical and administrative responsibilities:
Clinical Practice:
- Round consistently with the medical team
- Provide clinical pharmacy services and oversight for safe use of medication. This includes prescribing, dispensing, administering, and monitoring of medications. This may include, but is not limited to:
- Complete medication reconciliation
- Provide medication counseling
- Interpret pharmacokinetic/therapeutic drug monitoring data
- Screen for drug interactions, therapeutic duplications, omissions, etc.
- Monitor patients for therapeutic and/or toxic response to medications
- Coordinate patient care with the inpatient and outpatient transplant teams
- Ensure continuity of care and access to all medications
- Provide oversight of cost-effective medications usage and treatments
- Provide coordination and oversight of transition of high-acuity patients from inpatient to ambulatory
- Provide leadership in developing guidelines, order forms, and critical pathways to improve the care and safety of patients
- Provide guidance and educational support to all pharmacy staff providing support to patients
- Represent the transplant program's specific concerns for Information Technology applications
- Proactively assess and ensure medication adherence
- Develop and participate in pharmacy-related research projects and quality improvement initiatives, etc. within the program
- Provide oversight of transplant staff education for critical elements of medication use in patient care and clinical research
- Educate medical, nursing, and pharmacy staff about the management, prescribing and monitoring of transplant therapies
- Participate in the education of pharmacy students and PGY1 and PGY2 pharmacy residents
- Coordinate effort to provide timely drug information to the transplant program
- Present scholarly materials at educational conferences for the transplant program and pharmacy professional meetings and organizations (State/Local, National)
Administrative Practice:
- Participate in departmental and hospital committees and initiatives related to transplant
- Document clinical activities, consultations, education, etc. in the Electronic Medical Record
- Work with peers to achieve division and departmental goals
- Maintain familiarity with all departmental programs and work as an integral part of the transplant pharmacy team
- Model behaviors that exemplify Vanderbilt's CREDO
- Maintain outstanding skills as a clinical pharmacy practitioner and consistently demonstrate the value of pharmacy services
- Take responsibility for personal and professional growth
Position Qualifications
Required:

Doctor of Pharmacy or Master of Science
-
Tennessee pharmacist licensure
-
PGY1 Residency
-
PGY2 in Solid Organ Transplant or equivalent practice experience

At least 3 years of transplant experience post-PGY2 residency

Board Certification in Pharmacotherapy

Experience working in inpatient and outpatient settings in an academic medical center
